Learning Journal Artifact Reflection form - Rename it - relfections for project title, example - Reflections for Civil War project. Save in Reflections folder - document in Table of Contents
School Activities Log - save in Social folder

Faculty Forms / Information
Artifact Submission Form - This form is used to list all students who have met profiuciency on a given project and ready for uploading into portfolios.
Permission to Post Student Work Form - This form is signed by the student and department chair prior to posting student work on the portfolio website.
Portfolio Project Form for Department Chairs - This form is used to inform deparment chairs of porfolio projects planned for class.
Reflection Form - Access Directions - If needed, this form may be passed out to students. It provides printed directions to access the learning journal artificat reflection.
Upload Directions / Teacher Checklist - This document provides guidance in preparing classes to upload documents.
